What should I know about aFFF health records: diagnosis, treatment and exposure?
Keep a doctor's exact diagnosis separate from an AFFF exposure history. Learn which medical and work records make each part checkable.
Quick answer
A diagnosis and an exposure history are two different records. Medical files show what a clinician found, when it was found and how it was confirmed. Work, service, training and environmental files show where and when a person may have encountered AFFF or PFAS. Putting both timelines side by side does not decide causation, but it makes the facts easier to verify.

Use the diagnosis written in the chart
Do not rewrite a diagnosis into a broader word found online. Ask for the visit note, imaging report, pathology or biopsy report, operative report and discharge summary that carry the clinician's own language. Record the hospital, clinic and year. If you are unsure what was found, write that and identify the place that holds the report.

Build a second timeline for exposure
Keep job sites, bases, airports, academies, departments and residential addresses in date order. Add the task and the office that may hold proof. NIOSH describes firefighters as a worker group that may have higher PFAS exposure, but a group-level research finding does not tell what happened to one person. Individual records are what connect the timeline to real places and tasks.
