What should I know about how to request Depo-Provera and meningioma records?

Use the name the records office uses
A front desk may say it has no list of shots when the records office has a medication administration record. Ask for that term, plus medication lists, visit notes and billing records for the date range. For a scan, ask radiology for the report and DICOM image files. For surgery, ask health information management for the operative and pathology reports.

Track what arrived and what did not
Make one row per office with the request date, date range, records named, response and file location. A missing record should be marked missing, not silently treated as proof that an injection or visit did not happen. Old practices may have merged, closed or moved records to a custodian, so keep the original clinic name and address.
